The slab on my split level is heaving up and down throughout the year. Would a soil sample help me to determine the cause. If so how far down and how much?

The soil samples most labs analyze will not going to answer this question. Your local Soil and Water Conservation District may be able to assist if there are specific site issues to address, but you make need to have a contractor look at the site.
